Voluntary Special Needs/Persons with Disability Registration Program
The City of Bridgeport Health and Social Services Department has started a voluntary program that will allow individuals with Special Needs and Disabilities to register with the Health Department. This program is created for City of Bridgeport residents, who would need assistance for evacuation or emergency alerts (hearing impairment, sight impairment, bed confinement, wheelchair dependent, life support devices, use of TDD or other needs) that would hinder evacuation efforts.
This voluntary registration program is for the individual who is not in a skilled care facility, such as a hospital, nursing home or congregated living facility.
The information provided on the registration form will be kept confidential and only shared on an as needed basis.
Once you have enrolled into the program, Contact the Bridgeport Health & Social Services Department of any changes that may occur and affect your registration records. Please let us know immediately so that we can update your file.
The voluntary registration program will periodically update all the records.

Programa voluntario de Matrícula para la Población con Necesidades Especiales
La Ciudad de Bridgeport y el Departamento de la Salud y Servicios Sociales ha empezado un programa voluntario que permitirá a individuos con Necesidades Especiales e Incapacitados registrar con el Departamento de Salud. Este programa sé a creado para los residentes de Bridgeport, que necesitarían ayuda para la evacuación o en tiempo de emergencias.
Este programa voluntario de la matrícula es para el individuo que no está en una facilidad hábil del cuidado, tal como un hospital, cuidado en la casa o viviendo en una facilidad de asistencia.
La información proporcionada en el formulario de inscripción se mantendrá confidencial.
Una vez que usted se ha matriculado en el programa, Contacte el Departamento de Salud Publica y Servicios Sociales de cualquier cambio que puede ocurrir y poder afectar sus registros de matrícula.
El programa voluntario de la matrícula actualizará periódicamente todos los registros.
